We will also install any additional software requested/provided and install the necessary support tools and structures to allow us to provide efficient, effective, and affordable healthcare for your computer.House Call Onsite visit by IIS Staff. House calls are scheduled in advance with all HC4PC Clients.Office Visit Client drop-off of covered equipment at a HC4PC-approved site. Office Visits must be scheduled in advance with IIS Staff.Overnight Stay Occasionally, usually for Intake Exams/Conditioning or hardware repairs, we may need to keep your computer overnight at our office.CoPay The fixed fee per Office Visit or House Call set by you HC4PC Policy. Co-Pay amounts listed are per computer per incident.Monthly Premiums The monthly payment required to keep your HC4PC Policy in effect. Monthly Premiums are per computer and will vary based on the coverage level selected by Client for that specific computer. Premiums will be charged monthly to the credit/debit card on file for the Policy on the date specified by Client.Office Visit/House Call Hourly Rate The hourly rate for any services or care provided that is not covered under the computer’s HC4PC Plan. Hourly Rate charges are incurred in 15-minute increments with a minimum one-hour charge. Any Hourly Rate charges incurred must be paid in full at the time of completion unless otherwise authorized by IIS Staff.Telephone/Remote Support Hourly Rate The hourly rate for any telephone/remote support provided that is not covered under the computer’s HC4PC Plan. Any Phone/Remote Support Hourly Charges will be charged to the credit / debit card on file at the conclusion of the support call. If no credit/debit card is on file, a valid credit/debit card must be given at the beginning of the support call.Uninsurable Some computers may be too risky for HC4PC. Typically, these computers are older than 5 years and/or have too much physical damage to be insurable. To be considered for a HC4PC Policy, a computer must be capable of supporting 4 GB of memory and running a Microsoft operating system of Windows 7 or newer. Currently, HC4PC does not support Macintosh operating systems.Initial Installation House Calls for Initial Installation of covered computers are billed at the housecall hourly rate. New installation or replacement of networking hardware (i.e. modems, routers, switches, wiring, etc.) is NOT included in CoPay, and will be charged at the House Call Hourly Rate.Cancellation of Coverage HC4PC policies may be cancelled with 90-days advance written notification from Client; HC4PC Coverage will remain in effect for the 90-day period.HC4PC policies may also be cancelled immediately with written notification from Client accompanied by payment of the next three months premiums; HC4PC Coverage will terminate immediately.HC4PC policies will be cancelled immediately for non-payment if policy premiums are fourteen days past due. The policy will be reinstated immediately (within 90 days) upon receipt of past due premiums; any services performed on previously covered computer will be charged the non-covered hourly rate.Any HC4PC policy that has been cancelled for ninety days or more is ineligible for reinstatement. To initiate HC4PC coverage on the computer, payment of any past due amounts and a new policy with new Intake Exam/Conditioning and Initial Installation is required.Hardware/Software Purchases HC4PC does not sell hardware or software. We are happy to act as purchasing agent for our clients; a procurement fee may be applied. Unless other arrangements have been made, all hardware/software purchases will require advance payment in full. Appropriate documentation will be provided after the purchase has been made.Hardware/Software Requirements & Recommendations Every computer covered by HC4PC must have a valid Microsoft licensed operating system (Windows 7 or newer). Computers running operating systems that cannot pass the Windows Genuine Advantage test are not eligible for coverage. We will be happy to purchase a legal licensed Microsoft operating system on behalf of the client. Each year, IIS reviews the leading utility and anti-virus/anti-spyware software packages and selects a few that we will approve and support. Our selections are based on the strengths, merits, and weakness of the specific software package, and not on the brand, name, or previous product’s reputation. All HC4PC-covered computers must have a legal license for an IIS-approved utility software package and anti-virus/anti-spyware software package. For computers with ninety days or more remaining on a non IIS-approved utility or anti-virus/anti-software package, a pro-rated credit will be issued on the account. HC4PC Remote Support requires that the covered computer have a working connection to the Internet. Data Level 1 requires a one-time setup fee.Data Level 2 requires a one-time setup fee. An Uninterrupted Power Supply (UPS) is recommended for all covered PC’s and required for Data Level 1 and Data Level 2.Policy Riders Coverage for additional software packages (i.e. Quicken, Quick Books, Adobe Photo Suite, Adobe Elements, Microsoft Publisher, industry specific software, etc.) is available for an additional premium.Coverage for multi-media, entertainment centers, is available for an additional premium. Client Referral Program IIS does not advertise. We depend on the quality of our service and the referrals of our clients for new business and we believe that a win-win is the only way to do business. As an existing HC4PC policy holder, you will receive a credit equal to 5% of Initial Exams/Conditioning and 10% of monthly premiums received from your referrals.
